Actually, the key words in the M.E.’s report are hypertrophic cardiomyopathy (HOCM). This is a specific pathology (usually congenital) in which the fibres of the heart muscle are abnormally arranged, giving rise to a propensity to have spontaneous ventricular fibrillation (arrhythmic arrest) rather than infarction (‘heart attack”). Sudden, lethal arrhythmia is the commonest cause of death from those who have HOCM, but it’s usually missed until the PM is done, and undiagnosed HOCM is the commonest single cause of sudden unexpected cardiac death in young people. Stimulants, like cocaine, can precipitate VT arrest but would usually show up on a PM tox screen. In other words, Breitbart had it coming, but I’d still like to know what he was doing on the street at 23:00 .
